First Visit Checklist
What to bring.
A short list to make your first appointment as smooth as possible.
- Your insurance card (and the policy holder's date of birth if it isn't you)
- A photo ID — driver's license, state ID, or passport
- A current list of medications and supplements you take
- Prior dental records or X-rays if you have them on hand
- A note about anything you want us to know — anxiety, sensitivity, prior bad experiences
